Although investigation of aircraft mishaps is a vital part of the USAF mishap prevention program, these investigations are usually conducted by individuals with little or no experience in this field. This leads to errors in the investigations and the mishap reports. This report looks at two alternative systems in use today. The first used by the Federal Armed Forces, FRG and the second used by the US Army. The author then suggests a centralized program for the USAF. This program is based at the MAJCOM and is designed to meet Air Force needs and requirements. Recommendations for test program are also included.
