The Urban Mass Transportation Administration (UMTA) is conducting an investigation of the safety and security of the New York Metropolitan Transportation Authority (MTA) and its operating elements. The investigation is being conducted in two phases: Phase I Preliminary Investigation (PI), and Phase II Investigation. The document presents the findings and conclusions of the Phase I Preliminary Investigation. The investigation focused on identifying and assessing safety and security issues and recommendations associated with the operations of the MTA and its operating elements which have been previously identified by oversight agencies. The investigation also reviewed safety and security experience, and surveyed media reports concerning the MTA.
