The report explains the sinking of the passenger vessel Cougar off the coast of Oregon on September 15, 1988. The safety issues discussed in the report include Coast Guard policy for uninspected passenger vessels; watertight integrity; fatigue; emergency position indicating radio beacons; light attachments to life preservers; lifesaving equipment; Coast Guard inspection procedures for small passenger vessels; and Coast Guard standards for pyrotechnic distress signals. Recommendations addressing these issues were made to the United States Coast Guard.
