Mitigation of collision risk is critical for space operations safety and long-term sustainability. Robust mitigation approaches are based on actionable SSA products, performing conjunction assessment analysis, estimating collision risk, and conducting avoidance maneuvers. Beyond the typical components of conjunction processing, one is still left with questions concerning the accuracy and actionability of the results for a predicted conjunction. We examine factors that can affect and even negate the conjunction process, including force model errors, observation irregularities, and maneuver information. Aspects considered include positional accuracy, timeliness, and other relevant challenges. Key results conclude attention to detail is important, and importantly that covariance should not be used for conjunction calculations when future maneuvers are present. We also note the time required to re-establish the normal accuracy after a maneuver.
